INFORMATION
Appointments
Please call (03) 5222 4955 for an appointment during surgery opening hours. Every effort will be made to accommodate your preferred time. If the matter is urgent or you require a long consultation, please inform our receptionist. If you are unable to attend an appointment or you are running late, please ring and tell us. This helps us run to time. Please report to reception when you arrive. This practice complies with the Privacy Act and has taken all reasonable steps to protect the privacy of the personal information that we hold.
After Hours Consultations
For after hours medical attention, we use the National Home Doctor Service, who can be contacted on 137425. For more urgent medical attention the Geelong Hospital (University Hospital) in Ryrie Street provides a 24 hour public emergency department. St John of God Emergency Department is now open 24 hours 7 days per week and is accessed via the main entrance, 80 Myers Street (A fee applies approx $195.00). Epworth Geelong Emergency Department is open 8am-12midnight 7 days per week, 1 Epworth Place, Waurn Ponds - access via Pigdon Road (A fee applies approx $210.00).
Home Visits During Opening Hours
Home visits may be available at the end of consultation times for regular patients whose condition prevents them from attending the surgery.
Telephone Calls
Doctors from the practice may be contacted during normal surgery hours. The doctor may be available to speak to or a message will be taken and the doctor will return your call.
Test Results
Our Practice has a system in place for the follow up and review of clinically significant test results.
Repeat Prescriptions
From 1st December 2016, patients requiring a repeat prescription will need to book an appointment with your doctor.
My Health Record
An eHealth record is an electronic summary of your health records. You, and your healthcare providers, can access it online whenever you need, from wherever you are.
A My Health Record will allow healthcare providers access to patient information such as medications, test results, discharge summaries, allergies and immunisations. With the patient’s consent, this information can be quickly shared between healthcare providers involved in the patient’s care.
In addition, you can view your own healthcare information, enter personal information and keep notes for your own use.
Our staff can assist you with your registration for My Health Record.
Reminder System
Our practice is committed to preventative care. We may issue you with a reminder notice by SMS or letter offering you preventative services appropriate to your care. Please let us know if you do not wish to be a part of this system.
Your Rights
If you are not satisfied with our service, please contact us. We take complaints seriously and aim to resolve them quickly and fairly.
If you remain dissatisfied with our response, you may contact the Health Complaints Commissioner (HCC). The HCC responds to complaints about health services and the handling of health information in Victoria. Their service is free, confidential and impartial.

Your Personal Health Information
Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised personnel.
